Application to Enroll in ALMI (Archdiocese of Louisville Ministry Institute)  

Catechist Certification Stipend Incentive  (This request must come from the appropriate parish administrator; pastor, principal, PCL, DRE, etc.)

Program Approval for Catechist Credit Request Form (This form is to for requesting a program be approved for catechist credit.  This form can be used by an individual to request credit for themselves or for a host parish/school to request credit for their group of attendees.  This should be filled out in advance of the start of the program.)
